Retiree Dental Care Benefits
COBANC offers dental benefits for Retirees and their eligible dependents through the nationwide Capital Plan PPO Network. When outside of the New York/New Jersey area, COBANC Retirees also have access to an additional network known as the UnitedHealthcare PPO 20 Network.
Retirees who go out of our network for dental care benefits will be paid according to the schedule listed in the Retiree Supplemental Benefits Plan. To view plan coverage, please go to the Retiree Benefits Main Page.
Capital Plan PPO Network
COBANC Retirees have access to nationwide dental coverage through the Capital Plan PPO Network.
UnitedHealthcare PPO 20 Network
When outside of the New York/New Jersey area Retired COBANC members also have access to an additional national network of providers through the UnitedHealthcare PPO 20 Network. Copayments will apply and may vary based on the provider seen at the time of care. Please ask for a predetermination before getting any work done.
Locating a Provider
Please visit myuhc.com and register to set up your account. You will be able to locate a dental provider that participates in the COBANC Dental Plan. By registering, you will be able to keep track of your submitted and processed claims.
